초록
목적 : Hypoglossal neurilemmoma is very rare, reported 60 cases. Intracranial hypoglossal neurilemmoma reported in the literature presenting most commonly as a space-occupying lesion with symptoms of raised intracranial pressure. Removal of the intracranial hypoglossal neurilemmoma is possible via suboccipital craniectomy. We report our experience about intracranial hypoglossal neurilemmoma. 대상 : A 51-year-old female visited with a chief complaint of hypoglossal palsy, lower cranial nerve palsy, nuchal pain, which developed 5months ago. Initial MRI revealed an extra-axial mass in the posterior fossa bordering on the eroded occipital bone. It showed hypodense on T1-weighted images and hyperdense on T2-weighted images with enhancement. The medulla was displaced medially, and the mass extended extracranially along the right hypoglossal nerve through the eroded bone defect. 결과 : Tumor was totally resected with rt suboccipital craniectomy. It was a whitish-yellow and well-circumscrived, and several branches of the hypoglossal nerve adhered to the upper surface of the tumor, and extended into the enlarged hypoglossal canal. Histopathological examination of the resected tumor revealed it to be a neurilemmoma. She had no neurologic abnormality exception of the lt hypoglossal palsy and on review six months post-operatively, had recovered. 결론 : Hypoglossal neurilemmoma is rare and most is intracranial tumor. MRI should be included in the examination of the tumor, and greatly aids, in planning the total removal of the tumor. Suboccipital craniectomy is useful approach in intracranial hypoglossal neurilemmoma without extracranial extension.
