Study Design: Retrospective study. Objectives: The purpose of this study was to analyze the radiological outcomes of various cages, including three-dimensional (3D)-printed cages, polyether ether ketone (PEEK) cages, and metal cages with a 1-year minimum follow-up. Summary of Literature Review: In previous studies, the use of PEEK cages and metal cages showed good clinical outcomes. Recently, 3D-printed cages have been introduced to improve osteogenesis and osteointegration. However, no study has yet compared radiological outcomes between 3D-printed cages and other cages. Materials and Methods: The study was performed on 111 intervertebral spaces in 89 patients who underwent posterior lumbar interbody fusion (PLIF) using three types of cages at the L4-L5 and L5-S1 levels. Cage subsidence, intervertebral disc height, and the lordotic angle were assessed bon plain radiographs immediately after surgery and at 1 year postoperatively. Results: At the 1-year follow-up, several cases of cage subsidence were observed in the three groups. There were nine cases (25.7%) in the 3D cage group, eight cases (13.7%) in the PEEK cage group, and five cases (27.7%) in the metal cage group, but no statistically significant difference was found. Changes in intervertebral disc height and the lordotic angle also appeared not to be significantly different according to cage type. Conclusions: The 3D-printed cages used in PLIF showed similar radiological outcomes to those of PEEK cages and metal cages. Since PEEK cages and metal cages have shown satisfactory clinical outcomes and fusion rate, 3D-printed cages may have the potential to improve clinical outcomes in PLIF of the lower lumbar spine.
