Comparison of the Bone Graft Substitutes used in Posterolateral Fusion of Patients with Degenerative Lumbar Spine Disease

초록

Purpose: To compare the bone union rate in patients who had short-level (less than 2 level)posterolateral fusion for degenerative lumbar spine disease. Materials and Methods: From February 2008 to December 2011, 90 patients who had shortlevelposterolateral fusion surgery for degenerative spinal diseases were reviewed. We grafted autogenous iliac bone in 30 patients (Group I), autogenous iliac bone and demineralized bonematrix in 25 patients (Group II), and autogenous iliac bone and allogenous bank bone in 20patients (Group III). In 15 patients (Group IV), allogenous bank bone and demineralized bonematrix were used. The assessment of bone union was categorized by Lenke’s classification, andthen union rate of each group was compared respectively. Results: The fusion rate by Lenke’s classification among Group I, II, III and IV were 90.0%,84.0%, 85.0% and 73.3% respectively, and the difference was not statistically significant. Thetime to be confirmed of complete bone union were I(4.5 months), II(4.8 months), III(5.2 months),and IV(6.1 months). Group I showed earlier complete fusion than others. Conclusion: Fusion rate of each group using bone graft alternatives were lower than that ofgroup using only autogenous iliac bone (Group I), but there was no significant difference. Webelieve that allogenous bank bone and demineralized bone matrix could serve as useful bonegraft extenders in posterolateral spinal fusion.
