Inappropriateness of left ventricular mass is associated with left ventricular diastolic dysfunction, a study using tissue Doppler parameter of E/E`

Background: The structural significance of the inappropriateness of left ventricular mass(iLVM) has been reported to be an independent prognostic factor. But the functional changes associated with inappropriateness of left ventricular mass have not been established. This study was performed to investigate if diastolic dysfunction is associated with iLVM using tissue Doppler technique. Material and methods: 360 consecutive subjects including 141 hypertension, and/or 45 diabetes, and/or 45 hypercholesterolemia patients were analyzed for echocardiographic parameters. were E/A, DT, IVRT, and E/E`. The analysis was stratified with body mass index(BMI) of 25kg/m2. Observed/predicted LV mass(OPR) > 130 % was defined as inappropriate. Results: OPR was not independent factor determining any parameter among the transmitral blood flow. OPR( =0.146) was independent determinant of E/E` along with systolic BP( =0.114), age( =0.298), female( =0.158), and fasting blood glucose( =0.098). E/E` was higher in iLVM group(n=108) than appropriate LVM(n=252)(10.81 3.14 vs 11.82 3.50, p=0.009 adjusted for age, gender, systolic BP, fasting blood glucose). Conclusion: Inappropriateness of LV mass is independently associated with increased E/E` and E/E` was higher in iLVM group than aLVM group. These findings suggest that inappropriateness of LV mass is not only a structural change but also a functionally important change.
